This returned goods policy applies to the Everidis Health Sciences product line.

1. All Everidis Health Sciences product returns must be sent to:

   All returns must be accompanied by, and itemized on, a Product Return Form which may be obtained by calling Everidis Health Sciences toll free at 1-877-776-0101.
2. Product returned within 30 days of receipt by customer and meeting policy requirements will be processed without transaction or unit fees.
3. Everidis Health Sciences will issue credit or check for returned goods within 30 days of receipt in St. Louis. No deduction may be taken prior to that time.
4. Product not purchased either directly from Everidis Health Sciences or from an authorized wholesaler of the Everidis Health Sciences will not be accepted for credit.
5. Wholesalers are not authorized to accept return of Everidis Health Sciences products except in the case of mis-shipments or other ordering errors.
6. Returned products will be promptly evaluated and credit will reflect eligibility and/or ineligibility. All product returned, whether eligible for credit or not becomes property of Everidis Health Sciences. All Everidis Health Sciences products are returned with the understanding that they are subject to final review and evaluation by Everidis Health Sciences, and will be processed in accordance with the most recent returned goods policy in effect at the time the return is received.
7. Returned product will receive an exchange credit for value of merchandise returned less 10% of current wholesale price.
8. Credit will not be issued for product sold with the specific understanding that it is non-refundable, as specified in the price list.
9. This returned goods policy applies to the original purchase only. Product which has been repackaged, product purchased through a bankruptcy sale, fire sale, sacrifice sale or otherwise known as distress merchandise, or product not in the original container is not eligible for credit.
10. Everidis Health Sciences reserves the right to amend this policy by notification to the customer.
11. Retail customers should return product to place of purchase to receive credit.
